Register as a Patient

 

We are currently taking on new patients. We do occasionally take patients who live outside Exeter for various reasons, such as they work in Exeter.

 

Registering as a patient is simple. Download the form below, print it off and fill it in. You can also obtain a copy of the form from reception.

 

Once completed bring them along to one of our reception desks with proof of identity (i.e. passport, driving licence or ID Card) and we can register you.
